Telephone Health Coaching

Brief Overview

  • Earn 250 wellness points for coaching to help manage a chronic condition
  • Earn 200 wellness points for coaching to help make a healthy lifestyle change
  • Completion requirements: Participate in a minimum of three telephone sessions, although you are encouraged to complete additional sessions.
  • Deadline: Enroll by May 31, 2016, complete by August 31, 2016.

NextSteps Condition Management Telephone Coaching

Being healthy can be challenging, but managing a chronic condition often requires an extra commitment on your part. Get the support you need to manage diabetes, asthma, lung or heart disease by working with a health professional over the phone. if you qualify, you'll receive an invitation in the mail. Eligible participants who enroll no later than May 31, 2016, and complete a minimum of three phone calls by August 31, 2016, will earn 250 wellness points.

NextSteps Lifestyle Management Telephone Coaching

A health coach can provide unbiased information to help you develop a personalized plan to change behavior and help you set realistic fitness and nutrition goals for yourself. A health coach is available by telephone to all UPlan members and their covered spouses. Eligible participants who enroll no later than May 31, 2016, and complete a minimum of three phone calls by August 31, 2016, will earn 200 wellness points.

Starting with a Health Coach

As a UPlan member, you (or your spouse) have three ways to get started with a health coach.

  • If you want the advice and support of a health coach, you can telephone the University's provider of coaching services, StayWell Health Management. Call: 866-341-1170.
  • If the confidential results of the wellness assessment indicate that you or your spouse can benefit from health coaching, you will be invited online or over the telephone to enlist a health coach.
  • If an existing medical condition or disease is brought to the attention of coaches due to claims that you or your dependents submitted against your UPlan coverage, you will be contacted by an enrollment specialist.

To ensure your privacy, the University has contracted with StayWell Health Management, an independent provider based outside of the University to deliver health coaching services by telephone six days a week.

Face-to-Face Health Coaching

Depending upon where you work and live, face-to-face coaching may also be available for you or your UPlan-covered spouse/partner. In-person health coaching services are offered on campus in Duluth, Morris, and in the Twin Cities.
