Since you're newly eligible for the UPlan Medical Program, how you qualify for an annual rate reduction will depend on when your effective date is as well as when you enroll in benefits. Please see the following scenarios for your requirements.

|September 1, 2015 and after||September 1, 2015, or later||Can start earning wellness points for a 2017 rate reduction beginning on October 1, 2016.|
*Must enroll in benefits within the first 30 days of employment
The wellness assessment is a confidential online health questionnaire that provides immediate, personalized feedback about your risks for developing a serious medical condition or disease. The assessment is hosted online by StayWell, an independent third-party administrator.
You can sign in to StayWell Online directly to take the assessment using your existing University Internet ID and password. If you are the spouse or dependent on the UPlan you can access your StayWell Online account with this link.
Please note: As a new employee, StayWell will not have your information until about seven days into the month after you enroll in your benefits. For example, if you enroll in benefits on March 12, 2015, then you will be able to access your account on StayWell Online after April 10, 2015.
To complete the wellness assessment, you need to know your height and weight. Although you are welcome to input other health metrics if you know them, it’s not required. If you take part in a biometric health screening later on, your health metrics will be automatically entered into your assessment.
Boynton Health Service offers free health screenings on all campuses for UPlan members. You may also take part in a screening at your physician’s office and submit a form afterwards. For more information, go to the biometric health screenings page.
