Wellness Program for New Employees

Brief Overview

  • Earn 100 points: Take the online Wellness Assessment.
  • If you wish to participate in the Wellness Program and earn a premium reduction for the following year, refer to the chart below for enrollment deadlines.
  • The Wellness Assessment is hosted online by StayWell, an independent third party administrator for the Wellness Program. You will need to create a User ID and password using your seven-digit U of M employee ID number.

Wellness Assessment for Newly Eligible Employees

The Wellness Assessment is a confidential, online questionnaire that provides immediate, personalized feedback about your risks for developing a serious medical condition or disease.

As an employee newly eligible for the UPlan Medical Program, you can qualify for a premium reduction based upon completing the Wellness Assessment according to the following deadlines.

If you enroll in benefits between:Then you:
March 1 - May 31, 2014Must earn all 300 or 400 points and complete wellness assessment by August 31, 2014, deadline
June 1 - June 30, 2014Need only to complete wellness assessment by  August  31, 2014, to receive full 2015 reduction
July 1 - August 31, 2014Must enroll in benefits no later than August  31, 2014, and need only to complete wellness assessment by September 12, 2014, to receive full 2015 reduction
September 1, 2014, or laterCan start to earn wellness points for 2016 beginning October 1, 2014

Numbers Needed for Wellness Assessment

To complete the wellness assessment, you need to know your height and weight. Although you do not need the following test results to complete the Wellness Assessment, your assessment feedback will be more thorough and beneficial to you if you have them.

  • blood pressure
  • fasting cholesterol – total, HDL, LDL, triglycerides
  • fasting blood glucose
  • BMI (Body Mass Index)

Visit an on-campus biometric health screening to obtain screening results to help you gauge your health status.

Boynton Health Services offers free health screenings on campus for UPlan Medical Program members. Preventive screenings are also available at the UMD QuickCare Clinic. Please plan accordingly if you go to a Biometric Health Screening based on the deadlines.
