If you are enrolled in the UPlan Medical Program as:

*Employee required to earn a minimum of 300 points to achieve any reduction in premium; spouse/same-sex domestic partner (SSDP) can contribute up to 100 points. Participation by the spouse/SSDP is on a voluntary basis.

Weight Management

On-campus weight management

  • Weight Watchers® at Work. Minimum attendance: 14 meetings.
  • Create Your Weight (UMTC only). Minimum attendance: 9 classes.
 150 Registration fees returned when attendance requirements are met. Weight Watchers® at Work can be completed 3 times annually; 6 times lifetime. Create Your Weight can be completed just one time annually.

Healthy Pregnancy program

Step It Up!® walking program Receive a free pedometer. Length: 6 weeks.

Fit Choices dues reimbursement for exercise Register at a participating fitness center. Exercise 8 times/month to earn a $20 credit. Five monthly credits earned between January 1, 2012, and July 31, 2012, are required for 50 points.  50 Monthly $20 dues credit when you exercise 8 times/month. Enroll by March 1. Complete by August 31.
Bicycle commuter program Available early 2012.  50 $65 Complete by August 31.
Annual flu shot pledge Access online at: www.wellness.umn.edu. Self-report vaccination occurred within previous 12 months. Flu vaccine available at campus flu vaccination clinic: physician's office, retail convenience clinic.  25   Complete by August 31.
Non-tobacco user pledge (for 6 months) Access online at: www.wellness.umn.edu. Self-report tobacco-free for minimum of six previous months.  25   Complete by August 31.

PLEASE NOTE: Although it may be possible to complete an activity more than one time during the year, the point value associated with each of the programs listed above is the maximum number of points that can be earned for the completion of the activity in 2012.

3,4New employees who are benefits eligible between June 2, 2012, and October 1, 2012, and who take the wellness assessment by October 31, 2012, will qualify to earn wellness points to reduce their rate contribution for UPlan Medical Program coverage. New employees who are eligible for benefits after October 1, 2012, are required to wait until January 1, 2013, to earn wellness points.

5In lieu of biometric health screening data obtained through on-campus health screenings, UPlan members can instruct their health care provider to submit screening results using the  Biometric Health Screening Provider Form which can be accessed at: www.wellness.umn.edu
