The wellness assessment is a confidential, online questionnaire which assesses your health history and lifestyle habits. It provides immediate, personalized feedback about your risks for developing a serious medical condition or disease and can recommend lifestyle changes that are necessary to help you stay well.
The wellness assessment is open to employees, spouses or same-sex domestic partners, and dependents 16 years and older who are covered by the UPlan Medical Program. As an employee, you can take the 20-minute online questionnaire during your scheduled workday.
To begin the confidential wellness assessment, you will need to register. You will be asked to create a User ID and password by clicking on the "First Visit? Register Now" button. Even if you have taken the assessment in previous years, you will be required to create a brand new User ID and password when you login this year. Be prepared to provide your (or your partner's) seven digit (7) U of M employee ID number.

The wellness assessment will ask you to provide personal information to evaluate your overall health status. Although not required (except for providing your height and weight) the more information you can offer, the more thorough and useful the results will be to you. Information to provide includes:
To find out your numbers, visit a nearby clinic or a walk-in screening. On the Twin Cities campus, Boynton Health Service is offering once-a-month walk-in health screenings for UPlan members from now through December. Health screenings are available at no additional charge for UPlan Medical Program members and UPlan-covered spouses or same-sex domestic partners.
The deadline to earn $65 for taking the wellness assessment was April 30, 2009. However, if you are a newly-hired employee and eligible for benefits as of April 2, 2009, you will be able to earn a $65 wellness reward if you complete the wellness assessment by November 30. UPlan-covered dependents 16 years and older who take the assessment do not qualify to receive a $65 wellness reward. The $65 reward will appear in an employee's pay statement approximately three pay periods after the UPlan member completed the questionnaire.
After completing the wellness assessment, you and your spouse or same-sex domestic partner can each earn one more $65 wellness reward for the calendar year by completing one of the following health improvement programs during 2009:
