Alternative Format or Print Version of UPlan Wellness Assessment

A six-page printed version of the UPlan wellness assessment is available for UPlan Medical Program members and their dependents who have disabilities or who do not have easy access to a computer. Please contact StayWell Health Management at 1-800-926-5455 to obtain the printed questionnaire and an envelope to return your assessment via U.S. mail to StayWell Health Management.
