Health Coaching

Work with a health coach in person or through Next Steps over-the-phone to make long-term lifestyle changes. A health coach can develop fitness and nutrition goals tailored just for you.

Healthy Living Online Programs

Participate in a six-week, online program to learn about nutrition, fitness, healthy aging or disease prevention. There are 13 Healthy Living programs that offer a step-by-step approach to making behavioral changes, including Healthier Diet, Weight Loss, Becoming Smoke-Free, and Stress Relief. Earn 50 Wellness Points for participating; enroll by June 30, 2014, complete by August 31, 2014.

Medication Therapy Management 

Medication Therapy Management is a patient care service offered to UPlan members who take four or more medications for chronic conditions. With MTM, you have face-to-face meetings with a specially trained pharmacist who can help you improve the quality and effectiveness of your medication therapy program. New for 2014: UPlan members can receive 100 Wellness Points for meeting 3 times or more with a MTM pharmacist by August 31, 2014.

Specialty Therapy Management

New for 2014: UPlan members who need specialty medication to manage more complicated medical conditions are eligible to enroll in the Specialty Medication Management program offered by Fairview Specialty Pharmacy. UPlan members can receive 100 Wellness Points for a minimum of 3 telephone consultations with a Fairview specialty pharmacy nurse by August 31, 2014.

Treatment Decision Support

OptumHealth℠ Treatment Decision Support services are available to UPlan members who require consultations to help ease the stress of sorting through information when faced with a new diagnosis for a medical condition.

24/7 Nurse Line

Health and medical information is available from the OptumHealth℠ 24/7 NurseLine℠. Call the nurse line to get answers to your medical questions from a registered nurse at any time.
