Assess Your Health. Earn Wellness Points for Lower Premiums.

  • The wellness assessment is confidential, online questionnaire. It provides immediate, personalized feedback about your risks for developing a serious medical condition or disease.
  • The wellness assessment is open to employees, spouses or same-sex domestic partners, and dependents 18 years and older who are covered by the UPlan Medical Program. As an employee, you can take the 20-minute online questionnaire during your scheduled workday.
  • Take the wellness assessment by March 31, 2012. Earn 100 wellness points. Receive a $65 wellness reward. You and your spouse or same-sex domestic partner can each earn 100 points and each receive a $65 wellness reward.

Get the Most from the Assessment

Provide your blood pressure, cholesterol levels, blood glucose level, and BMI (Body Mass Index), and the more useful your assessment feedback will be.

Visit an on-campus biometric health screening to obtain screening results to help you gauge your health status. Boynton Health Services offers free and conveniently-located health screenings on campus for UPlan Medical Program members. Preventative screenings are also available at the UMD QuickCare Clinic.

Take the Assessment

Take the Wellness Assessment, which is hosted by StayWell Health Management, an independent third party.

  • First-time Participants:

    You will need to create a User ID and password using your (or your spouse/partner's) seven-digit U of M employee ID number.

  • Returning Participants:

    You will log-in using the User ID and password you previously created, this is, provided your password is not more than 100 days old.

Earn Points in 2012 for Lower Medical Premiums in 2013

Wellness points accumulated in 2012 can be used the following year, in 2013, to reduce the cost of UPlan Medical Program coverage for you, your spouse/same-sex domestic partner, and your family. Your premium reduction will be $300 or $400 when you have earn the required minimum number of points -- 300 or 400 depending upon the level of health care coverage you chose during open enrollment. In 2012, you and your spouse/partner can also earn a $65 wellness reward when you complete the wellness assessment no later than March 31, 2012.

After completing the wellness assessment (and undergoing biometric health screenings), you and your spouse or same-sex domestic partner can each earn additional wellness points and another $65 wellness reward by completing one of the following:

PLEASE NOTE: Although it may be possible to complete an activity (such as the wellness assessment or a Healthy Living program) more than one time during the year, the point value associated with each of the wellness program offerings is the maximum number of points that can be earned for the completion of an individual activity in 2012. Again, the deadline to earn wellness points and a second $65 wellness reward is August 31, 2012.

Numbers Needed for Wellness Assessment

To complete the wellness assessment you will need to know your height and weight.

When you also provide the following test results, your assessment feedback will be more thorough and beneficial to you.

  • blood pressure
  • fasting cholesterol – total, HDL, LDL, triglycerides
  • fasting blood glucose
  • BMI (Body Mass Index)
  • height and weight (required for assessment)
