Biometric Health Screenings

Earn 100 points to add to your Wellness Points when you participate in biometric health screenings no later than August 31, 2014. When you earn 300 or 400 wellness points (depending upon the tier of UPlan Medical Program coverage that you elected) you can reduce your 2015 premiums by $300 or $400.


Health Screenings for Twin Cities UPlan Members at Boynton

Faculty, staff, and their spouses or same-sex domestic partners (SSDP) who are members of the UPlan Medical Program are encouraged to attend a preventive health screening administered by Boynton Health Service.

Remember: To earn 100 points,you must complete your screening by August 31, 2014. While you may undergo screenings more than one time in a year, 100 points is the maximum numbers of points that can be earned for completing a biometric health screening in 2014. The test results data will be electronically transmitted by Boynton directly to a participant's online wellness assessment hosted by StayWell Online, the University's independent third party administrator for the Wellness Points program.


Preventive Screenings at: UMD QuickCare Clinic, Physician's Office, or Health Care Provider

Depending upon the location of your work place, there are various sites you or your family members can visit to obtain your biometric test results.

Regularly-scheduled medical exams and health screenings are considered preventive care and their cost, in general, is covered by your medical plan. Ask your physician or medical provider what tests are appropriate for you. As part of routine recommended care, all adults should have their height, weight, and body mass index (BMI) recorded every year.

Additional tests, such as blood cholesterol or blood glucose, can be combined with a doctor's visit. Download the “Know Your Numbers” form (pdf) and use the form to record your biometric information.

In lieu of biometric health screening data obtained through an on-campus screening, UPlan members can instruct their health care provider to submit screening results using the Biometric Health Screening Provider Form. This form is also available through StayWell Online.

Numbers Needed for Wellness Assessment

To complete the wellness assessment you will need to know your height and weight.

If you can also provide the following test results, your assessment feedback will be more thorough and beneficial to you.

  • blood pressure
  • fasting cholesterol – total, HDL, LDL, triglycerides
  • fasting blood glucose
  • BMI (Body Mass Index)
  • height and weight (required for assessment)
