Beginning in January 2012, when you complete a wide range of health and wellness activities you can earn points for your Wellness Points Bank. The wellness points you earn next year can be used to reduce (by $300 or $400) the price you pay for UPlan Medical Program coverage in 2013.

The wellness assessment is a confidential, online questionnaire which assesses the status of your health. Employees and their spouses or same-sex domestic partners who are covered by the UPlan Medical Program can earn 100 points and a $65 wellness reward when they complete the questionnaire between January 3 and March 31, 2012. You can accumulate an additional 100 points when you participate in biometric health screenings no later than August 31, 2012.

When you or your spouse/same-sex domestic partner are covered by the UPlan Medical Program, you can select from a number of programs designed to help you feel better and live longer. Complete one of the following options to earn points and a $65 wellness reward:
You and your spouse are invited to participate in a weight management program on your campus to earn 150 points for your Wellness Points Bank. When, as a UPlan member, you attend the required number of meetings or classes, you will get back 100 percent for the up-front costs of the program. During a calendar year a UPlan member can be reimbursed three (3) times for taking part in weight management programs.
