  1. Letters of transmittal from chancellors, vice presidents, deans, or directors, as applicable, expressing concurrence or disagreement with the recommendation of the review committee(s) with an explanation of their views
  2. A statement from the appointing official summarizing the accomplishments of the candidate and a statement as to whether he or she concurs with the recommendation of the review committee and the basis for concurrence or disagreement.
  3. Statement of the respective unit's standards, policies, and procedures for continuous appointment. Stated criteria must be consistent with job responsibilities of candidate. List specific examples of the performance that led to meeting a specific criterion.
  4. A complete and current curriculum vitae to include:

    1. undergraduate, graduate or professional, and, if applicable, postdoctoral education and experiences with respective institutions, locations, dates, and degrees awarded;
    2. former and present employment with years and dates of service at the University and present personnel status
    3. professional distinction such as research and scholarly activities, participation in professional organizations, and awards, citations, and other recognitions; (d) professionally related services within the University, the community, the State, and elsewhere; and
    4. special contributions to departmental, collegiate, and University functions. Where activities are listed, specify the results of these activities; e.g., if a candidate serves as a committee member, state some of the accomplishments made by that committee and the role of the candidate's contributions on the committee.
  5. Current position description for the individual candidate.
  6. Information from unit review committee(s) relevant to the voting procedures to include

    1. number of majority votes, minority votes, abstentions, and eligible persons not voting due to absence;
    2. review committee(s) recommendation based upon the majority view with a statement of the grounds upon which the recommendation rests; and
    3. a statement of any minority view that has substantial support and the basis for such a position.
  7. Evaluations from distinguished individuals in the profession outside the University regarding the candidate's professional activities (include copy of letter sent to external evaluators).
  8. Evaluations from peers within the University regarding the candidate's professional activities (include copy of letter sent to internal evaluators).
  9. Where applicable, evaluations from the users of the service performed by the candidate.
  10. Current annual appraisal (President's Form 26 "Appraisals of Probationary Academic Professional Staff") and copies of annual appraisal forms from prior years.
