Procedure

College, Campus, or Administrative Unit

  • Obtain the Retirement Incentive Option for Faculty and Academic Professional and Administrative Staff materials from the Employee Benefits Web site.
  • Certify employee eligibility and determine years of service.
  • Provide information to participating employees regarding the age distribution of eligible employees within unit and job class (pdf).
  • Obtain necessary signatures on the Retirement Incentive Option for Faculty and Academic Professional and Administrative Staff Agreement and, on the last day of employment, the Retirement Incentive Option for Faculty and Academic Professional and Administrative Staff Release and provide benefits information.
  • Enter appropriate information into the HRMS system.

Employee Benefits

  • Verify division/department calculation of years of service.
  • Assist division/departments when questions arise concerning eligibility and determination of years of service.

Employee

  • Complete and sign the Retirement Incentive Option for Faculty and Academic Professional and Administrative Staff Agreement and the Benefits Election form.
  • Return the Retirement Incentive Option for Faculty and Academic Professional and Administrative Staff Agreement and the Benefits Election form to your department. Your application must be handed in to the responsible administrator/supervisor by September 26, 2008, or no later than the effective date of retirement, whichever occurs first.
  • Sign the Retirement Incentive Option for Faculty and Academic Professional and Administrative Staff Release on your last day of employment. (A Release signed prior to the last day of employment will not be accepted.)
  • Send the Release to Employee Benefits. Your benefits under this program will NOT begin until the signed Release has been received by Employee Benefits.
  • Contact the Employee Benefits Service Center at (612) 624-9090 or (800) 756-2363 (option 2) with questions.

This procedure should be followed in a timely manner to avoid a delay in continuation of insurance coverage.
