Campuses:
Each campus/area has a base medical plan with the lowest rate and out-of-pocket costs. You may choose from any of the plan options available in your area. The rate you pay depends on who you cover and which plan you choose. If you are employed 75-100% time, the University pays a significant portion of the cost.
Prime Therapeutics is the pharmacy benefits manager for all UPlan medical plans. Fairview Specialty Pharmacy is the exclusive provider of most specialty medications. You are automatically enrolled in the pharmacy program when you enroll in a medical plan.
Each campus/area of the state also has a base dental plan. You may choose from any of the plan options available in your area. The rate you pay depends on who you cover and which plan you choose. If you are employed 75-100% time, the University pays a significant portion of the cost.
The University provides you with a basic life policy equal to your salary, rounded to the next $5,000 with no maximum. For example, if you make $46,000, your life insurance amount would be $50,000; however, if your salary is $45,000, your life insurance amount remains at $45,000. Faculty and P&A staff with salaries of $95,000 or less have an additional life benefit under the basic life insurance. The University pays the premium if your appointment is 75% or more, but you may purchase this coverage if your appointment is 50-74%.
