UP! Circles Application Form

There is a $50 fee to cover materials and program costs. A very limited number of waivers will be available. Waiver process.

Materials are due Friday, January 11, 2013. The program steering committee will review applications and contact applicants regarding their confirmation to participate by January 16. Cohort will be limited to 50 participants.

Incomplete applications will not be considered.

The application information provided below will assist the Program Steering Committee with matching small groups. Please provide information about your professional and personal background and interests. As you complete the application, try to limit your responses to 100 words or less where applicable.
