Departments have the responsibility to provide COBRA information to employees. Review these documents for information on those responsibilities.
Please complete the COBRA Verification form and mail the original to Employee Benefits.
Departments on the Twin Cities Campus may request the Guide for UPlan Benefits Enrollment and New Employee Orientation Invitations through an online form.
When making your request, please note that the New Employee Orientation Invitations are printed for six-month periods (January through June and July through December).
