Summary of Benefits

The Summary of Benefits booklets provide a detailed description of the coverage you have for medical and dental benefits in the University of Minnesota UPlan.

These booklets explain the eligibility provisions of the UPlan, events that can cause you to lose coverage, your rights to continue coverage when you or your dependents are no longer eligible to participate in the UPlan, and your rights to appeal a coverage decision or claim denial.

Medical Coverage

The 2013 UPlan Medical Summary of Benefits booklet has descriptions of the medical plan options, benefit charts that describe each plan option's coverage, the pharmacy coverage for prescription medications, and the additional benefits that are part of your medical plan.

Notice about Prescription Drug Coverage and Medicare

The Centers for Medicare and Medicaid Services (CMS) regulations require that this notification is available to all individuals with prescription drug coverage who are eligible for Medicare.
