Base plan for Twin Cities and surrounding counties

It is an affordable, network-only plan. The Delta Dental PPO network includes more than 1,300 participating dentists in 76 counties in Minnesota and border communities.

Under this plan you must visit a dentist participating in the Delta Dental PPO network to receive benefits. With the exception of out-of-area emergencies, benefits will be denied if you do not visit a Delta Dental PPO dentist. You do not need to select a primary care dental clinic when you enroll.

Delta Dental Premier

Base plan for Greater Minnesota

It is a flexible plan that offers access to the broad Delta Dental Premier network as well as the more cost-effective Delta Dental PPO network. Out-of-network benefits are also available. The Delta Dental Premier network is Minnesota's largest dental network — with more than 2,500 dentists in 86 Minnesota counties and border communities.

Visiting a dentist in either of the Delta Dental networks will help you make the most of your benefits and can result in out-of-pocket cost savings. You do not need to select a primary dental clinic when you enroll.

University Choice

University Choice is administered by Delta Dental and offers freedom to see any provider of your choice. There may be a payment difference between the allowed reimbursement and what the dentist charges for a particular service, which could result in greater out-of-pocket costs for you.

Visiting a dentist who participates in the Delta Dental PPO or Delta Dental Premier networks may result in lower out-of-pocket costs, in addition to having the $50 deductible waived under this plan.

UPlan HealthPartners Dental

It is an affordable, network-only plan. Under this plan you must visit a dentist participating in the network to receive benefits. This plan's network has more than 840 dentists and specialists at over 590 dental clinics in 67 Minnesota counties and border communities. You do not need to select a primary care dentist when you enroll.

UPlan HealthPartners Dental Choice

It is a broad network plan with out-of-network benefits. Visiting a dentist in the HealthPartners network will help you receive the highest level of benefits. The network for this plan has more than 1,500 dentists and specialists at over 1,000 dental clinics in 67 Minnesota counties and border communities. You do not need to select a primary care dentist when you enroll.
