Life Insurance

Will Preparation Services

Using established legal formats, the will preparation services provided by LifeWorks allow you to create simple wills, financial power of attorney, living wills, or final arrangements.

Services include a 30-minute consultation with an attorney by phone or in person to review completed forms or consult on other legal matters. You can purchase additional legal services beyond 30 minutes at a 25 percent discount.

You can access the tools for creating documents and the network of attorneys at www.LifeWorks.com.

The user ID is “will” and the password is “preparation.”

The University provides basic group term life insurance to all benefits-eligible employees. The basic life insurance amount is one times your salary, rounded up to the next $5,000. There is no maximum. If your appointment is 50% to 74% time, the insurance amount is still based on the full annual salary. Basic employee life is provided to you regardless of your health history.

The amount of basic life insurance automatically doubles in the event of death by accident.

To Enroll

If you are employed at least 75% time, your enrollment in this plan is automatic at the time you become eligible for UPlan benefits. However, if you are employed 50% to 74% time, you will need to enroll.

When you are first eligible, within the first 60 days of employment or benefits eligiblity, Employee Benefits will send you information on how to enroll.

Effective Dates for Basic Benefits

Premiums

The University pays the premium for basic life if you are employed at least 75% time.

If you are employed 50% to 74% time, you pay $.06 per $1,000 coverage per pay period based on your full annual base salary rounded up to the next $5,000.

Additional Optional Insurance

You also have the opportunity to purchase additional employee life insurance, insurance for your spouse/same-sex domestic partner, and for your children.

Additional Life Insurance

Reduction Schedule

There is a reduction schedule in basic life insurance if you are age 67 or older. At age 67, the amount is reduced to 65%; at age 70, it is reduced to 50%; and at age 75, the life insurance amount is reduced to 25% of the original benefit.

Accelerated Benefit Options

Both the basic and optional insurance plans have an accelerated option. This means if you or a family member become terminally ill, you may be able to collect the full amount of the life insurance benefit prior to death.
