| The Head and Neck Anatomy CD-ROM may only be purchased via a site license. The site license includes (with all three units) an instructor's copy of the CD-ROM and copies of the CD-ROM for each student enrolled in your anatomy course. A personal copy of the CD-ROM will give your students choices as to how, when and where they learn the material. The student may use the CD-ROM as a review tool throughout their curriculum. The site license fee would be billed annually to your school (see subscription order form for fee). Many schools recoup this fee by charging their students for the CD-ROM since it will be theirs to keep. | |||
| View Site License Agreement and Order Form. The Order Form is a PDF file for use with Adobe Acrobat Reader. It allows you to type in the fields and then print a copy to be mailed or faxed. Please be aware that you will be unable to save your changes to the document and may wish to print an extra copy for your own records. |
